Before Your Appointment

A little preparation goes a long way. Here is what we recommend before your visit to DESSWO.

Lab Work

If your provider ordered labs prior to your visit, please complete them at least 3 days before your appointment so results are available for review.

Medication List

Bring a complete, up-to-date list of all medications you take, including supplements, vitamins, and over-the-counter drugs with dosages.

Blood Sugar Log

If you monitor your blood sugar at home, bring your glucose meter or a printed log of recent readings. This helps us tailor your care plan.

Prior Records

Any previous lab results, imaging reports, or specialist notes related to your condition are helpful. Request these from your previous providers in advance.

Insurance & ID

Bring a valid photo ID and your current insurance card. If your plan requires a referral, make sure it is in place before your visit.

Questions to Ask

Write down any questions or concerns you want to discuss. Appointments go smoothly when you come prepared with what matters most to you.
